Slávek Horák (lahir 12 Januari 1975) adalah seorang sutradara, penulis latar dan aktor Ceko. Ia adalah lulusan Sekolah Film Zlin dan juga belajar penyutradaraan di Sekolah Film Praha FAMU.

Ia juga berkarya sebagai asisten sutradara dengan Jan Sverak pada film pemenang penghargaan Academy Award buatannya “Kolja” (1996). Pada 2012, ia memulai perusahaan produksinya sendiri TVORBA films. “Home Care (film)” adalah debut fiturnya. Film tersebut terpilih sebagai perwakilan Ceko untuk Film Berbahasa Asing Terbaik di Academy Awards ke-88.[1][2][3][4]
