Meropenem
Meropenem is een breed-spectrum antibioticum dat behoort tot de klasse van de carbapenems, die effectief zijn tegen een breed scala aan bacteriën, waaronder veel multiresistente stammen. Het middel wordt voornamelijk gebruikt voor de behandeling van ernstige infecties veroorzaakt door bacteriën die gevoelig zijn voor carbapenems. Meropenem is sinds 1994 op de markt en is verkrijgbaar onder de stofnaam.
Chemische eigenschappen
Meropenem is een β-lactamantibioticum met een karakteristieke structuur die zorgt voor een breed werkingsspectrum. Het bevat een bèta-lactamring, die essentieel is voor de antimicrobiële activiteit. Meropenem is gevoelig voor de hydrolyse door de meeste β-lactamase-enzymen, maar wordt beter beschermd tegen veel van deze enzymen dan andere β-lactam-antibiotica, zoals penicillines.[1][2]
Biologische eigenschappen
Meropenem remt de synthese van de celwand van bacteriën door te binden aan penicillinebindende eiwitten (PBP's).[1] Het medicijn wordt meestal toegediend via intraveneuze of intramusculaire injectie. De stof wordt voornamelijk geklaard door de nieren.[1][3]
Indicaties voor het gebruik van meropenem zijn:
- ernstige longontsteking
- onderste luchtweginfectie bij cystische fibrose[4]
- gecompliceerde urineweginfecties, intra-abdominale (in de buik gelokaliseerde) infecties, huidinfecties en weke delen-infecties
- bacteriële meningitis
- perinatale (rond de geboorte) infecties
- bacteriëmie (als gevolg van een van bovenstaande infecties)[1]
